Ergonomic Matching Funds

 

The ERM Ergonomic Program focuses on ergonomic modifications of individual employee computer workstations.  Matching funds are available for furniture or accessories recommended as a result of a computer workstation evaluation provided by the ergonomists. 

The following are the steps to take to qualify for matching funds: 

1. Request an ergonomic evaluation. E-mail drive-ergo@sfsu.edu or call 415-338-2565. 

    A workstation evaluation is required to qualify for matching funds. 

2. Purchase the recommended furniture and accessories.

**Furniture and accessories purchased remain the property of the department making the purchase. 

Matching funds are only available for furniture or accessories recommended as a result of a computer workstation evaluation provided by the ergonomists. 

ERM will match 50% up to $250** per employee for recommended furniture or accessories.

 

How To Apply?

Complete the SF State Fiscal Affairs Department ‘Transfer Payment Request’ form. 

Attach a copy of the invoice(s) or receipt(s) for purchased furniture and/or accessories, along with the FDW (Financial Data Warehouse) report that corresponds with posted payment and send to Laura Lyons, Administrative Analyst, llyons@sfsu.edu

Upon receipt, ERM will transfer 50% of the purchase price up to $250** per employee to the purchasing department. 

Funding for this program is limited. 

Applications will be considered on a first-come, first-served basis.

Funding reimbursement will be delayed or denied for paperwork that is incomplete or with insufficient supporting documentation.
