Special Event Questionnaire

This form must be completed for all SF State sponsored/hosted events or events taking place on SF State property. Please submit this form at least 14 business  (two weeks) before the event date to allow for sufficient time to process requests. 

If alcohol will be served at an event, an alcohol clearance form is required. This form must be completed and signed by a representative of the sponsoring organization, the Chief of University Police, and the Campus Risk Manager.

If the event is taking place off campus, please provide the full address of the location(s).
This question specifically applies to minors (individuals ages 17 or younger) who will be participating in an activity where their legal guardian(s) will not be present. Checking "Public Event" implies that the invitation is open and there may be minors present with their families.
