The Driver Safety Program authorization forms are available as templates in DocuSign.
- Driver Safety Program Application & Authorization Form – This form should be submitted by those wishing to drive State and rental vehicles on university business. Once received, applicant will be provided with a link to online driver safety training. The approved driver must renew this authorization every four years.
- STD 261 Authorization to Use Privately Owned Vehicles on State Business – This form should be submitted by those wishing to driver their personal vehicle on university business. The approved driver must renew this authorization annually.
- STD 261 Authorization to Use Privately Owned Vehicle Renewal Form – This form should be re-submitted annually by those previously authorized to drive their personal vehicle on university business.
The list of authorized drivers and the expiration date of their authorization can be found on the ERM website’s Driver Safety Program webpage.
